Comply users have the ability to link together different form types and create a unique link to be sent to people.
For instance, you want the Asset, Consent and Immunisation Forms to be linked and send as one. Below are the steps to guide you.
Note: You can only create a composite form if you have existing forms already.
Step 1: Click Employment Self-Service in Settings.
Step 2: Click the plus button under the Self-Onboarding heading to add forms.
Step 3: Select Composite in the Form Type drop-down. Click Add.
Step 4: Start creating your Composite Form.
A.Enter the name for the form.
B. Provide a description (optional).
C. Select the User Verification: either form is for Default User (People already in Comply) or Asset Form (For New People in Comply)
- If you choose Asset Form, you need to choose which Asset Form from the drop down.
D. Select the first type of form (Consent/Immunisation/Dynamic). Ensure that you also have chosen the form from the drop down associated with that type.
E. Click the plus button to add next type of form. Repeat Step D.
F. Press Save.
* Should you wish to delete the form type, click the (-) icon on the top right side.
Step 5: Your form will be shown in the Employment Self-Service Forms List. Copy the link and send it to your worker to commence filling out the Composite Form.
