This guide will show you how to create an immunisation form using the OnePassport Management System.
Capture the required immunisation of your workers before commencing their employment using the Employment Self-Service feature of OnePassport Management System. Simply send the link to your worker to fill out and submit it to your organisation.
Important: Prior to creating the Immunisation form, you may wish for your workers to confirm consent in collecting immunisation related records. You may collate these two forms and send thru one link.
1. Click "Settings"
Click on the "Settings" option.
2. Click Employment Self-Service Form
Click on the specified location.
3. Click Create button
Click on the specified location.
4. Click the Form Type Dropdown and select Immunisation
Click on the specified option.
5. Click "Create"
Click on the "Create" button.
6. Provide a "Name" for the Immunisation Form
Click on the "Name" field.
7. As for the Immunisation Types section, tick the checkboxes for the Immunisation Types you want to be included
Click on the specified button.
8. Under the Fields section, the required ones are already pre-selected. Should you wish to require an additional field like File, simply tick the check box
Click on the specified button.
9. Click "Submit"
Click on the "Submit" button.
10. Click "Ok" on the success pop up message
Click on the "Ok" button.
